Abstract
The term computer-based patient record (CPR) first used in 1980s, reflects some of the first attempts to automate essential functions such as medication, administration, and order communication. In 1990s, the term Electronic Medical Record (EMR) was used to describe a system that integrate transcription and document imaging with previous CPR functions. this study is to determine the effectiveness of Electronic Health Records in the health care delivery. The study will help the healthcare providers to exchange health information electronically, to provide higher quality and safer care for the organization through effective use of electronic health record across the hospital. Electronics health record has the ability to generate a complete record of a clinical patient encounter as well as supporting other care related activities directly or indirectly via interface including evidence based decision support, quality management and outcome reporting
